PHYSICALS
GSPI strongly urges all participants to obtain
a physical examination from a licensed physician
prior to beginning any physical conditioning
program. Under certain circumstances, GSPI
may require a Physician’s approval prior to
beginning a training program.
PAYMENT INFORMATION
All payment for services shall be made payable
in immediate funds to:

You have the option of paying for the session in
one payment
or you can make in 2 payments.
Payments not received by the 10 days after the
due date or 7 days after enrolling in class will
incur $20.00 late fee. A $30.00 Returned check
fee will be assessed for returned checks
Refunds/ Make up Classes
Godspeed does not offer refunds for missed classes.
Once you register for a session you are obligated
to attend and pay for the entire session. If
you know your child will miss a class please
contact our office within 24 hours of the scheduled
class by email or phone and we will offer you
a make up class in the next session. Each child
will only be allowed 2 make-ups per session.
CLASS ATTIRE
Please make sure your student has proper athletic
shoes and loose fitting athletic clothes for
class. Cleats are not necesary as we will be
training on artificial turf.
HYDRATION
A cold bottle of Gatorade or water sent with
them to class will provide much needed hydration
as we are working. Athletes will be allowed to
take water breaks throughout the training.

I hereby grant GSPI and their respective licensees,
successors and assigns, the right and permission,
with respect to those photographs or videos
taken of the minor named below on whose behalf
I am signing, and with respect to any printed
matter on connection therewith, to do the following:
To include such photographs in all the media.
And in advertising, publicity, and promotion
thereof.
I hereby certify that I am the parent and/or
guardian of
a minor
under the age of 18 years, and hereby consent
on behalf of said minor to the use of any of
the photographs or video taken of said minor
pursuant to the terms set forth in this Photographic
Release, including, without limitations, the
release, discharge and hold harmless provisions
thereof
